We are very blessed to be able to provide our ministry volunteers, that are unable to attend weekly services (Worship, Family Church School, Bible Study, or Special Event), with complimentary media (CDs or DVDs).

Please use the form below to request complementary media.  This form is required to initially establish your weekly demand.  It is not necessary to resubmit this form each week unless your demand (+ or -) changes.

Please help us to be good stewards.  Please include your requirement only for volunteers that have missed a weekly service, because they were serving during that service.    All (*) items are required.

• Note: All requests (Family Church School & Worship Service) submitted by Thursday 12 noon, will be ready for pick-up by the following Sunday in the Media Room. Mid-Week Services and Special Events will be available a week from Sunday.
