Vehicle Use Request Form

Resurrection Baptist Church members may request use of church-owned vehicles for approved activities. The form below must be submitted at least 10 days prior to your event. All (*) items are required.

This form must be submitted for vehicle use. Should your event be cancelled, please contact the church office within 24 hours.

It is your responsibility to ensure the following:
1. Remove all belongings and place in church Lost & Found.
2. Empty all trash and return the vehicle clean.
3. Inform maintenance supervisor of any problems with vehicle incurred during activity.
4. If you are returning vehicle(s) on Saturday afternoon or later, ensure that gasoline is full.
Future use of all church vehicle(s) depends on how vehicles are returned after initial use.
